PHYSICAL FITNESS & WELLNESS PROGRAM

1. PURPOSE Stormtech encourages a proactive approach to maintaining a healthy lifestyle and provides a physical fitness and wellness benefit. 2. SCOPE This policy applies to all regular full-time and part-time Stormtech associates working in all locations that have completed three (3) months of continuous employment. 3. PROGRAM • The Physical Fitness and Wellness Program provides $125 CAD reimbursement towards eligible programs, equipment, and activities, per annum. • Programs, equipment, and activities that are eligible under the program include: o Membership fees to a health club or fitness center. o Sport footwear or hiking shoes that associates use for their sporting activities. o Wellness related programs (i.e., smoking cessation, weight watchers, nutrition counselling etc.). o Fitness devices or equipment. o Instructional classes at a fitness facility, virtual or remote (aerobics classes, yoga, Tai Chi, personal training etc.), that are at least 6 weeks in duration, or 6 classes. o Sports league/activity-based programs of at least 6 weeks in duration (i.e., swimming, baseball, hockey, softball, volleyball, dance classes, etc.). o Home office equipment (i.e., router, computer monitor, mouse keyboard, webcam, computer (desktop or laptop), headphones or headset, office chair, lumbar support, footstool, desk or standing desk, printer). • Home office equipment is only reimbursable to employees that are eligible to work remotely. (If you require further clarity as to your eligibility, please speak to your manager or the People & Culture department) • Weight loss or meal replacement supplements, clothing, strictly recreational activities (poker clubs/leagues) are not eligible for reimbursement, and products or services deemed non-taxable by the CRA. • This program can only be used for Stormtech employees’ personal fitness and wellbeing, it does not apply to dependents. • Reimbursement is a taxable benefit and will be processed through Payroll with all applicable taxes. It will appear on your T4 as a taxable benefit.

4. PROCESS How does the Physical Fitness and Wellness Program Work? •

Employees pay for their expenses personally, then submit receipts and the form for reimbursement. • Complete the Physical Fitness & Wellness Reimbursement Form and forward it to People & Culture for processing, along with the original receipt(s). The form is available from People & Culture or on Confluence. • Reimbursement Forms can be submitted once per year for all receipts at any time throughout the year, for purchases made during that year.
