5. PROCESS All Educational Requests •

Prior to registering, eligible associates must submit the Continuing Education Request Form and provide necessary documentation for the request to their manager. Documentation includes course or program description, fees, time offerings, purpose/reason of request. It is recommended to discuss the request with your manager as well prior to completing the forms. • The Department Manager and People & Culture will review each request for final approval and involve the Department Vice President as required. Short Certificate or Training Classes • Upon approval, the associate must submit the invoice to Accounts Payable in the Finance Department for payment. • The associate must complete registration with the institution. Extended Courses and Designation Programs • Upon approval, the associate must submit the Repayment Agreement Form to People & Culture prior to registering for the course. • The associate must complete registration with the institution. • Upon successful completion of the approved course, the associate must submit documentation of the passing grade and receipts for relevant expenses to People & Culture for reimbursement. • If the associate decides to leave Stormtech within the timeframes set out below, they will be responsible for repayment of fees and expenses as follows for any reimbursed expenses in excess of $1000.00 (per expense):
