SICK AND PERSONAL DAYS

1. PURPOSE Stormtech provides paid time off for associates to attend to personal matters and when ill or sick. 2. SCOPE This policy applies to all Stormtech Associates, including part-time, temporary, or casual employees working in Canadian locations who have completed ninety (90) consecutive days of employment. 3. POLICY A. Sick Days • Stormtech associates are provided five (5) days per “calendar year” for any personal illness or injury. • The sick leave balance is reinstated to the five (5) days the next employment year. Sick days are not carried forward into the following year. • Sick days may be used after 90 days of consecutive employment, based on date of hire. • Doctor’s notes may be requested from time-to-time for sick-related absences, i.e. after three (3) consecutive days’ absence or in the case of frequent short-term absences. B. Personal Days • Stormtech associates are provides with two (2) days per “calendar year” for other personal matters (i.e.: religious observance, moving day, appointments, etc.). • The personal leave balance is reinstated to the two (2) days the next calendar year. Personal days are not carried forward into the following year. • Personal days may be used after 90 days of consecutive employment, based on date of hire. C. Process • Days may be taken in one (1) day increment or half (½) day increments. • Associates must notify their immediate manager, or in the absence of their manager, notify his/her designate at least one (1) hour prior to starting their shift to report an absence or as soon as they become aware of the requirement. • Doctor’s notes may be requested from time-to-time for sick-related absences, i.e., after three (3) consecutive days’ absence or in the case of frequent short-term absences. Sick and Personal Days are inclusive of provincial allowances for Family Responsibility Leave (BC) and Personal Emergency Leave (ON). Any leaves still permitted by provincial legislation beyond the six (6) days per annum provided for in this policy will be unpaid. Managers are responsible for • Ensuring time off is approved and appropriately accounted for on forms and timesheets. Associates are responsible for • Notifying their managers of required absences, completing appropriate forms, and providing the necessary medical documentation as requested. 4. RESPONSIBILITIES
