A request for flexible working should be made in writing to your line manager and should set out: • the date of the request; • the fact that it is a request for flexible working; • the change that is being requested (e.g. to working hours, times of work, place of work, etc.); • the date from which you wish the change to take effect; and • if and when you have made a previous request for flexible working. N.B. Any change agreed under this section would be a permanent change to your terms and conditions of employment and you would not have the right to revert back to your original working pattern. Any change which resulted in a reduction in hours would also result in a commensurate reduction in pay. Consideration of the request Your Line Manager will consider the request and will either agree to the changes you have requested or arrange a meeting in order to discuss it further. Your Line Manager will write to you as soon as possible, and normally within 14 calendar days of the meeting, informing you of their decision. 17.Medical and other appointments You are normally expected to ensure that appointments to visit the doctor, dentist, hospital, etc. are made in your own time and outside normal working hours. In the event that this is not reasonably practicable, time off work will be permitted to attend such appointments providing that the appointment is substantiated with an appointment card (if requested) and the timing of the appointment causes as little disruption as possible i.e. at the beginning or end of the working day. Normally in such circumstances you will be required to make up the time missed whilst attending the appointment. Alternatively, paid time off may be granted at the discretion of the Company. 18.Compassionate Leave You are entitled to up to three days paid leave in the event of the death of an immediate family member. Additional unpaid leave may be considered, depending on the particular circumstances. Immediate family means your spouse, common-law partner, civil partner, child, parent, sister, brother, mother-in-law, father-in-law, grandmother, grandfather, grandmother-in-law, grandfather-in-law or any relative permanently residing in your home.
